What is Remote Patient Monitoring?

  • Remote patient monitoring (RPM) is a form of telehealth that uses digital medical devices to collect and transmit health data from patients outside of clinical settings1,1,2. 


RPM allows providers to monitor and manage chronic conditions, modify care plans, and provide education on self-care. RPM may increase access to care and decrease healthcare delivery costs

Clinical Pharmacist Involvement

CP ARNPs, pharmacists and technicians have two main goals in mind when partnering with you: 

To provide additional patient care by helping to fill the gaps between regular appointments:

  • Improve MIPS/MACRO monitored outcomes for better reimbursements. 
  • Reduce ER and hospitalizations
  •  Reduce staff time for patient calls related to HTN, HF, COPD/Asthma, diabetes


To provide your staff with additional medication resources, information, and provider care coordination

  • We ship and set up new patient devices. 
  •  Real time notification of biometric measurements out of range:

  1. can help prevent complications and unnecessary admissions
  2. encourage and educate patient on proper disease management. 
  3. Notification sent to managing provider


  • Monthly report of patient data with a summary of any phone calls from CP. 
  • Patient services are billed to their insurance monthly. Most patients have no financial responsibility. 
  • Managing providers have the option to be the supervising physician of record to review and bill under their NPI with CP. 


Monthly Remote Patient Monitoring (RPM) process

Remote patient monitoring is the monitoring of biometric data in real time using electronic transmission of data from telemetric devices. 

  •  Technicians educate patients on how to properly use their devices and monitor daily readings. 
  •  Patient are required to have 16 reading in a 30 day period for coverage by Medicare. 
  •  Should a reading be outside of the parameters prescribed by the provider or should a patient not take readings over other day:
  • The RPM technician will check in with the patient to either verify that the reading were properly obtained or encourage the patient to take readings routinely. 

  1.  If the reading was confirmed to be properly taken and outside of acceptable parameters, our CP ARNP will check in with the patient by telephone to provide additional review of the patient
  2.  Evaluate situation for face to face care requirements and encourage patient follow up with regular provider in a timely manner to avoid complications
  3. Provide timely education to the patient regarding their medications, symptoms and potential complications.


Things to Consider

  • We  are more than happy to provide verified medication lists (pharmacy and/or providers), to be uploaded to the patient’s chart and can be used to confirm what the patient should have at home. Prior to first visit or any time in between 
  • We are available to contact providers or pharmacies concerning medication related problems that may come to your attention during a visit
