Easy for you - Easy for your patient

  •  Your Clinical Pharmacist has access to Your EMR and monitors it daily for new patients or patients who have not had a pharmacist visit yet
  •  Our pharmacist will send the nurse a message as soon as she identifies the patient to call to discuss patient specifics
  • We will call the patient within 2 days of hospital discharge or admit to service.
  • After proper consent and explanation of services, we will conduct a medication review, evaluate for RPM, identify education or compliance barriers
  • Follow up will be done at least monthly and is based  on RPM and medication compliance needs

  We work synergistically with your team to compliment your staff and extend patient care to improve patient outcomes, minimize staff time and optimize population health management.

Important Points

As patient needs are identified, the pharmacist will work with your nurse 

The pharmacist will send all communications to the nurse to relay to the physician

The pharmacist can act as a pharmacy liason and financial advisor: 

  • send refills
  • compliance package medications
  • arrange delivery
  • assess for de-escalation
  • review medication insurance for coverage
  • help patients apply for financial assistance (if available)

We can adjust this at any time based on your preferences and needs
